Controversy remains over whether the coronavirus disease 2019 (COVID-19) virus may have infectivity during the incubation period before the onset of symptoms. The author had the opportunity to examine the infectivity of COVID-19 during the incubation period by conducting an epidemiological survey on a confirmed patient who had visited Jeju Island during the incubation period. The epidemiological findings support the claim that the COVID-19 virus does not have infectivity during the incubation period.

잠복기 동안 제주도를 방문했던 중국인 COVID-19 확진자에 대한 역학조사를 수행하였다. 환자와 직접 접촉한 11명을 14일간 격리조치 하였는데, 이들 모두 격리해제까지 COVID-19 감염증상을 보이지 않았다. 이상의 역학조사 결과는 잠복기때는 감염력이 없다는 주장을 뒷받침한다.

Objectives
The purpose of this study was to determine whether eating non-home-prepared meals (NHPM), including fast food, ready-to-eat foods, and frozen foods, was associated with self-reported infertility in the United States women. Methods: Data on diet and infertility from women aged 20-49 years who participated in the 2013-2014 and 2015-2016 National Health and Nutrition Examination Surveys were analyzed (n=2143). Dietary information, including the number and types of NHPM consumed, was obtained from a self-reported questionnaire, and infertility status was analyzed using the following question, “Have you ever attempted to become pregnant over a period of at least a year without becoming pregnant?” Results: The frequency of NHPM consumption was positively associated with self-reported infertility after adjusting for confounding effects (odds ratio [OR], 2.82; 95% confidence interval [CI], 1.48 to 5.38 of >1 vs. 0 NHPM/d). The odds of infertility were 2-3 times higher in women who consumed fast food than in those who did not consume fast food (OR, 2.73; 95% CI, 1.15 to 6.48 of >1 vs. 0 times/d). Conclusions: The frequency and types of NHPM may be a factor contributing to infertility. Although our findings require confirmation, they suggest that eating out may be deleterious to women fecundity.

본 연구에서는 미국 국민건강영양조사 자료를 사용하여 패스트푸드, 냉동식품, 그리고 포장하여 판매하는 음식과 같이 집에서 조리하지 않은 음식 섭취와 불임의 연관성을 분석하였다. 잠재적인 교란변수들을 통제하여 분석한 결과 집에서 조리하지 않은 음식 섭취 횟수는 불임과 연관이 있는 것으로 나타났다 (OR 2.82, 95% CI 1.48 to 5.38 of >1 vs. 0 NHPM/d).

Prescription of Systemic Steroids for Acute Respiratory Infections in Korean Outpatient Settings: Overall Patterns and Effects of the Prescription Appropriateness Evaluation Policy

Objectives
The objective of this study was to identify individual and institutional factors associated with the prescription of systemic steroids in patients with acute respiratory infections and to investigate the role of a policy measure aimed to reduce inappropriate prescriptions. Methods: We used data from the National Health Insurance Service-National Sample Cohort from 2006 to 2015 and focused on episodes of acute respiratory infection. Descriptive analysis and multiple logistic regression analysis were performed to identify individual-level and institution-level factors associated with the prescription of systemic steroids. In addition, steroid prescription rates were compared with antibiotic prescription rates to assess their serial trends in relation to Health Insurance Review and Assessment Service (HIRA) Prescription Appropriateness Evaluation policy. Results: Among a total of 9 460 552 episodes of respiratory infection, the steroid prescription rate was 6.8%. Defined daily doses/1000 persons/d of steroid increased gradually until 2009, but rose sharply since 2010. The steroid prescription rate was higher among ear, nose and throat specialties (13.0%) than other specialties, and in hospitals (8.0%) than in tertiary hospitals (3.0%) and other types of institutions. Following a prolonged reduction in the steroid prescription rate, this rate increased since the HIRA Prescription Appropriateness Evaluation dropped steroids from its list of evaluation items in 2009. Such a trend reversal was not observed for the prescription rate of antibiotics, which continue to be on the HIRA Prescription Appropriateness Evaluation list. Conclusions: Specialty and type of institution are important correlates of steroid prescriptions in cases of acute respiratory infection. Steroid prescriptions can also be influenced by policy measures, such as the HIRA Prescription Appropriateness Evaluation policy.

이 연구에서는 요양급여 청구자료를 기준으로 급성 상기도 감염에서 스테로이드를 처방하는 경우와 관련하여 다음과 같은 특징을 확인하였다. 첫째, 환자 특성과 기관 특성을 모두 고려하였을 때 진료과목과 기관의 종별 구분에서 감기 스테로이드 처방률은 상당한 변이를 보였다. 둘째, 약제급여적정성평가 항목에서 스테로이드가 제외된 직후 처방률이 시계열적으로 상승하였으며, 이는 처방 행태에 정책 요인이 중요한 영향을 미칠 수 있음을 뜻한다.

Objectives
The quality of life (QoL) of the elderly and elder abuse are growing public health concerns. This study aimed to evaluate the prevalence of abuse and the association between QoL and abuse in older adults.
Methods
Multistage random cluster sampling, along with valid QoL (LEIPAD: LEIden, the Netherlands; PADua, Italy; Helsinki, Finland) and abuse questionnaires, were used to assess QoL and elder abuse. Path analysis was performed using Mplus. SPSS and AMOS were used for the other analyses.
Results
A total of 386 elderly individuals with a mean age of 68.00±6.10 years were interviewed, of whom 200 (51.8%), 289 (74.9%), and 376 (97.4%) were women, educated, and married, respectively. Moreover, 167 (43.2%) had low-to-moderate QoL, and 108 (27.9%) had experienced a moderate level of abuse. QoL and abuse were inversely associated (r=-0.253), with men (β=-0.24) more affected than women (β=-0.21). Musculoskeletal disorders were also strong determinants of QoL in the elderly. QoL was strongly associated with emotional abuse, while abuse was highly related to the social component of QoL. Furthermore, emotional abuse was the type of abuse most significantly associated with the self-care, depression/anxiety, cognitive, and social components of QoL. Sexual abuse, violation of personal rights, and neglect were the main determinants of the physical functioning, life satisfaction, and sexual domains of QoL, respectively.
Conclusions
Nearly half of the elderly individuals lacked a high QoL, and at least one-fourth had experienced some form of abuse. Elder abuse was correlated inversely with QoL. Therefore, preventive interventions are recommended to decrease elder abuse in the family, community, and other settings.

Objectives
Obesity is regarded as one of the most prominent health threats worldwide and a serious risk factor for non-communicable diseases, such as diabetes mellitus type 2, high blood pressure, cardiovascular diseases, and some types of cancer. Given the role that societal development—as reflected by the Human Development Index (HDI)—may play in the prevalence of obesity and overweight, this study aimed to investigate the degree to which the prevalence of obesity and overweight is affected by HDI and its components.
Methods
In this ecological study, the required data on HDI and its components were gathered from the latest report of the United Nations Development Program, and data on obesity and overweight were acquired from the latest reports published on the World Health Organization website. Statistical analyses were conducted using SPSS version 24.0.
Results
The prevalence of obesity was determined to be significantly higher among females than males, and the gross national index per capita was found to be significantly higher for males than females (p<0.05). Significant positive correlations were found between HDI and its components and sex, as well as indices of obesity and overweight.
Conclusions
A significant positive correlation exists between HDI and obesity. As policy-makers attempt to improve the general welfare of the people, they should be aware of potential unwanted effects of development on the risk of obesity and overweight among the population.

Objectives
This study aimed to develop a valid social contact survey instrument and to verify its feasibility for use among Korean adults.
Methods
The Delphi technique was used to develop an instrument to assess social contacts, which was then applied in a cross-sectional pilot study. A panel of 15 medical professionals reviewed the feasibility and validity of each item. The minimum content validity ratio was 0.49. Thirty participants used the developed measure to record contacts during a 24-hour period.
Results
After a systematic review, the survey instrument (parts I and II) was developed. Part I assessed social contact patterns over a 24-hour period, and part II assessed perceptions of contacts in daily life and preventive behaviors (hand hygiene and coughing etiquette). High validity and feasibility were found. In the pilot study, the 30 participants had a combined total of 198 contacts (mean, 6.6 daily contacts per person). The participants’ age (p=0.012), occupation (p<0.001), household size (p<0.001), education (p<0.001), personal income (p=0.003), and household income (p<0.001) were significantly associated with the number of contacts. Contacts at home, of long duration, and of daily frequency were relatively likely to be physical. Assortative mixing was observed between individuals in their 20s and 50s. Contact type differed by location, duration, and frequency (p<0.001).
Conclusions
The developed social contact survey instrument demonstrated high validity and feasibility, suggesting that it is viable for implementation.

본 연구에서는 감염접촉전파에 대한 인식과 사회적접촉을 측정 도구를 델파이 방법으로 개발하여 국내최초로 한국 성인의 사회적접촉을 측정하였다. 참여자 30명은 24시간 동안 총 198명을 접촉하였으며(6.6명/일/인), 접촉자수는 연령 (p=0.012), 직업(p<0.001), 가구수 (p<0.001), 교육수준 (p<0.001), 개인소득 (p=0.003), 가구소득 (p<0.001)에 따라 차이가 있었다. 동질성 혼합(assortative mixing)은 20대와 50대에서 보였으며, 접촉유형은 접촉장소, 접촉기간, 접촉빈도에 따라 유의하였다(p<0.001). 향후 국내 사회적 접촉 양상과 특성에 관한 전국적인 후속연구를 제언한다.

Objectives
This study investigated the role of information and communication technology and women’s empowerment in contraceptive discontinuation in Indonesia.
Methods
The study used data from the 2017 Indonesia Demographic and Health Survey and monthly contraceptive calendar data. A Gompertz proportional hazards model was used for analysis.
Results
The 12-month contraceptive discontinuation rate was higher among women who had used the Internet in the past year, women who were mobile phone owners, and women who reported having fully participated in household decision-making than among their counterparts. These factors significantly impacted the risk of contraceptive discontinuation in Indonesia, even after controlling for contraceptive method, age, parity, contraceptive intent, education, work status, place of residence, and wealth status.
Conclusions
After adjustment for the control variables, a higher risk of contraceptive discontinuation was associated with having used the Internet in the past year, owning a mobile phone, and not participating in household decision-making. Higher contraceptive discontinuation risk was also associated with using contraceptive pills, older age, lower parity, intent of spacing births, more education, current unemployment, and rural residence, and the risk was also significantly higher for those in the lowest household wealth quintile than for those in the fourth household wealth quintile. The association of contraceptive discontinuation with the use of modern information and communication technology and relatively disempowerment in household decision-making may imply that information regarding family planning and contraception should be conveyed via social media as part of setting up an eHealth system. This must include a strong communication strategy to empower and educate women in contraceptive decision-making.

Objectives
Cancer diagnoses have a tremendous impact on individuals and communities, drawing intense public concern. The objective of the current research was to examine news coverage and content related to cancer-related issues in Korean newspapers.
Methods
Primarily using the database system of the Korea Press Foundation, we conducted a content analysis of 2806 articles from 9 Korean daily newspapers during a recent 3-year period from 2015 to 2017. Thematic categories, the types of articles, attitudes and tone, and the number of sources in each article were coded and classified.
Results
Many news articles dealt with a diverse range of themes related to cancer, including general healthcare information, the latest research and development, specific medical institutions and personnel, and technology and products, which jointly accounted for 74.8% of all articles. Those thematic categories differed markedly in terms of article type, tone, and the number of cited sources. News articles provided extensive information about healthcare resources, and many articles seemed to contain advertising content. However, the content related to complex social issues such as National Health Insurance did not include enough information for the reader to contextualize the issues properly or present the issues systematically.
Conclusions
It can be assumed that the media exert differential influence on individuals through news coverage. Within the present reporting framework, the availability and usefulness of information are likely to depend solely on individuals’ capabilities, such as financial and health literacy; this dependency has a negative impact on knowledge gaps and health inequities.

Objectives
In recent years, multiple studies have investigated the issue of intimate partner violence (IPV) in Korea. However, most of those studies have focused on IPV against women, while overlooking the problem of men IPV victimization. Considering this, the current study identified risk and protective factors for IPV and examined their influence on IPV victimization among Korean men.
Methods
We used a nationally representative sample of 1668 Korean men from the 2013 Korea National Survey on Domestic Violence. The associations between potential IPV risk factors and different types of IPV were investigated using univariate and multivariate logistic regression. Specifically, separate analyses were conducted of 5 types of IPV (neglect, controlling behaviors, emotional violence, economic violence, and physical violence).
Results
The prevalence of IPV among Korean men and women showed only marginal gender differences. Controlling behaviors (men, 23.3%; women, 23.9%) and emotional violence (men, 16.5%; women, 18.8%) were the most common types of IPV reported, followed by neglect (men, 11.2%; women, 11.7%). Separate logistic regression analyses for the 5 subtypes of IPV revealed that mutual IPV was a strong predictor of IPV. Men who abused their wives were more likely to experience neglect (odds ratio [OR], 29.24; p<0.01), controlling behaviors (OR, 36.61; p<0.01), emotional violence (OR, 58.07; p<0.01), economic violence (OR, 18.78; p<0.01), and physical violence (OR, 38.09; p<0.01).
Conclusions
The findings of this study suggest that IPV intervention strategies should particularly focus on couples whose relationship is characterized by patterns of bidirectional violence.

Objectives
We calculated life expectancy and inequalities therein by income for the period of 2016-2018 across the 253 electoral constituencies of the 20th National Assembly election in Korea.
Methods
We obtained population and death data between 2016 and 2018 from the National Health Information Database and constructed abridged life tables using standard life table procedures according to gender and income quintiles for the electoral constituencies of the 20th National Assembly election held in 2016.
Results
Life expectancy across the 253 constituencies ranged from 80.51 years to 87.05 years, corresponding to a gap of 6.54 years. The life expectancy difference by income across the 253 constituencies ranged from 2.94 years to 10.67 years. In each province, the difference in life expectancy by income across electoral constituencies was generally greater than the inter-constituency differences. Constituencies in capital and metropolitan areas showed a higher life expectancy and a lower life expectancy difference by income than constituencies in rural areas.
Conclusions
Pro-rich inequalities in life expectancy by income existed in every electoral constituency in Korea. Both intra-constituency and inter-constituency socioeconomic inequalities in health should be highlighted in future policy-making in the National Assembly.

이 연구는 2016년 치뤄진 제20대 국회의원 선거구 253개에서 2016-2018년의 기대수명을 산출하고, 소득 상위 20%와 소득 하위 20% 간 기대수명 격차를 제시하였다. 연구 결과, 253개 모든 선거구에서 소득 상위 20%가 소득 하위 20%보다 기대수명이 높게 나타났으며, 선거구 내의 소득에 따른 기대수명 격차가 선거구들 간의 기대수명 차이보다 더 큰 경향을 보였다. 기대수명의 선거구 간 격차를 줄이고, 선거구 내의 소득 계층 간 기대수명 격차를 해소하기 위한 입법적 조치를 마련하는 데에 이 연구의 결과가 근거자료로 유용하게 사용되기를 기대한다.
